Your Benefits Committee has deemed the following items noteworthy:

-  It is vital to keep your contact information current.  Important benefits information is sent to you through the mail system and it may include a time sensitive date for action. 

-  Part-time is scheduled to begin in March.  If you downgrade to part-time between the 1^st and 15^th of the month, your coverage will end on the 15^th at midnight, if you downgrade to part-time between the 16 and the last day of the month, coverage will end on the last day of the month at midnight.  You may continue your coverage under COBRA.  Information about your rights under COBRA will be mailed to your home of record. 

-  The company will continue to provide Basic Life and Accidental Death and Dismemberment (AD&D) coverage. 

-  Short Term Disability (STD), Long Term Disability (LTD) and Supplemental Life will remain in effect if you have already enrolled in these programs.  However,  you will have 30 days from the date you downgrade to part-time to cancel or decrease those benefits, remember it is based on your part-time earnings.  

-  To cancel or decrease your STD, LTD or supplemental life insurance go to www.eelect.com and enter the enrollment id #12645.  You will have 30 days from the date you downgrade to part-time to make changes. 

-  The 401K Plan eligibility will remain the same for part-time. 

-  The Pass Benefits will remain the same for part-time. 

-  If you use prescription maintenance medicine, please use the mail order method to save money.  The form is located in the FA lounge.  Complete your portion of the form and attach the prescription and mail it in with the payment.
